What These Clinical Cases Demonstrate

Understanding Your Investigation Reports

If you have been advised surgery for an ovarian tumour, your reports may contain terms such
as:

Ultrasound

CT/ MRI

These investigations estimate the likelihood of ovarian cancer but cannot accurately determine the final stage. Comprehensive surgical staging remains essential.

Understanding Your Blood Test (Tumour Marker) Report

Your doctor may advise one or more of the following tumour markers before surgery:

An abnormal tumour marker does not confirm ovarian cancer, and a normal result does not completely exclude it. These tests help estimate the possibility of malignancy and guide surgical planning.

Common Questions Before Surgery

My ultrasound says "complex ovarian cyst." Is it cancer?

Not necessarily. Many complex ovarian cysts are benign. However, some may represent borderline ovarian tumours or early ovarian cancer. Further evaluation by a gynecologic oncosurgeon is recommended.

My MRI mentions "papillary projections." What does this mean?

Papillary projections increase the suspicion of an ovarian tumour and require specialist evaluation. They do not confirm cancer but usually warrant surgical assessment.

My CT scan says "no enlarged lymph nodes." Is that good?

Yes, this is reassuring. However, CT scan cannot detect microscopic disease. Comprehensive surgical staging may still be required if ovarian cancer is suspected.

My report says "no ascites" and "no omental deposits."

These are favourable imaging findings and may suggest that the disease appears localized. However, the final stage can only be confirmed during surgery and histopathological evaluation.

My CA-125 is high. Does this mean I have ovarian cancer?

Not always. CA-125 may be elevated in ovarian cancer as well as endometriosis, fibroids, pelvic infection and other benign conditions. It should always be interpreted together with imaging findings.

My CA-125 is normal. Can I still have ovarian cancer?

Yes. Some women with early-stage ovarian cancer have normal CA-125 levels. A normal tumour marker should not delay appropriate evaluation when imaging is suspicious.

My doctor says I need staging surgery. What does that mean?

Staging surgery determines whether cancer is truly confined to the ovary. Depending on the individual case, it may include assessment of the omentum, peritoneum, pelvic lymph nodes and para-aortic lymph nodes to ensure accurate diagnosis and treatment planning.

Why are lymph nodes examined if my scan is normal?

Small areas of microscopic disease may not be visible on CT scan or MRI. Assessment of pelvic and para-aortic lymph nodes helps determine the true stage of disease.

Why is the omentum removed during surgery?

The omentum is one of the common sites where ovarian cancer can spread microscopically. Its assessment is an important part of comprehensive staging surgery.

Can fertility be preserved in Stage I ovarian cancer?

In carefully selected young women with early-stage disease, fertility-preserving surgery may be possible without compromising oncological safety. The decision depends on tumour type, stage and individual patient factors.

Can Stage I ovarian cancer be treated by robotic surgery?

Selected women with apparent early ovarian cancer may be suitable for laparoscopic or robotic staging surgery, provided complete oncological principles can be achieved. Patient selection is more important than the surgical technology used.

Should I consult a gynecologic oncologist before surgery?

Yes. If your ultrasound, CT scan, MRI or tumour markers suggest a suspicious ovarian tumour, consultation with a gynecologic oncologist before the first surgery helps ensure appropriate planning and may avoid incomplete surgery or the need for a second operation.

Key Take-home Messages

Imaging Suggests, Surgery Confirms

Ultrasound, CT scan and MRI can identify a suspicious ovarian tumour, but they cannot accurately determine the final stage of ovarian cancer. Comprehensive surgical staging remains the gold standard.

Common Report Terms

  • Complex adnexal mass
  • Papillary projections
  • Thick septations
  • Solid-cystic ovarian lesion
  • CA-125
  • HE4
  • No ascites
  • No omental deposits

The First Surgery Matters

The first operation is the most important opportunity to achieve accurate staging, complete tumour removal and appropriate treatment. Proper planning may reduce the need for repeat surgery.

Why Choose a Gynecologic Oncosurgeon?

Specialized care includes comprehensive staging, frozen section-guided decisions, fertility preservation when appropriate, minimally invasive surgery in selected patients and evidence-based ovarian cancer management.
