IN SHORT

Yes, for some women. Chemotherapy goes first. If the disease then shrinks enough for all visible cancer to come out, an interval operation is offered.

Stage IV means cancer cells have reached beyond the abdomen. That may be fluid around a lung, or deposits inside the liver or other distant organs.

Strength shapes the plan as much as the stage. A weak or underweight woman starts with chemotherapy, and her strength is checked again before any operation.

This page is one part of a larger guide. For who needs surgery for advanced ovarian cancer, how it is planned and what the stages mean, see advanced ovarian cancer surgery (stage 3 and 4) in Ahmedabad.

IVA and IVB: what the letter tells you

Ovarian, tube and peritoneal cancer share one staging system. Stage IV is the furthest spread. It splits into two groups, and the letter changes how the spread is checked.

The two parts of stage IV
GroupWhat has been found
IVA.Cancer cells in the fluid around a lung.
IVB.Deposits inside an organ such as the liver, or outside the abdomen.

Even here, the abdomen often holds most of the disease. That is why an operation can still matter. Our staging page explains every stage.

First, confirming it

A label of stage IV on a scan is not enough to start treatment. These checks come first:

  1. Tissue proof. A core biopsy or a laparoscopy confirms the cancer before chemotherapy. A needle aspirate or fluid test alone is not enough.
  2. The chest fluid. If there is fluid around a lung, it is sent to look for cancer cells.
  3. A PET-CT, when spread outside the abdomen is suspected or unclear. It is not a routine test in ovarian cancer.
  4. The starting point. If the markers point away from the ovary, a colonoscopy and upper endoscopy check the bowel and stomach.
  5. The BRCA and HRD test, sent now. It decides the medicine that follows chemotherapy.

Why chemotherapy leads in stage IV

In stage IV the usual path is chemotherapy first, then an operation where it can remove all visible disease, then the rest of the chemotherapy.

The standard drugs are carboplatin and paclitaxel, one cycle every three weeks. Weekly paclitaxel is used if the standard plan is hard to tolerate. In stage IV, bevacizumab is added from the second cycle. It continues alone afterwards, for up to fifteen months. It is never started within four weeks of an operation, and is paused six weeks before one.

When an operation is still offered

After three or four cycles, the tumour board looks again. A laparoscopy with a spread score, or an open look, shows what can be removed. A scan alone does not decide it. The interval operation goes ahead only if no visible cancer will be left. Our page on complete removal or chemotherapy first explains this rule.

What usually happens in each situation
SituationUsual next step
Fit, and the disease has shrunk enough.Interval operation, then the remaining cycles.
Fit, but not all the cancer could be removed. No partial operation. Chemotherapy continues.
Weak, low albumin, or older with other illnesses.Chemotherapy first, then a fresh check of strength.

Heated chemotherapy (HIPEC) has its proof at the interval operation in stage III. Whether it has any place for you is a question for the tumour board. Our cytoreductive surgery page covers the operation itself.

After chemotherapy: keeping it in check

Maintenance medicine starts within twelve weeks of the last cycle. The genetic test picks it: olaparib for a BRCA change, other choices when the result is different.

A BRCA change usually means olaparib tablets for two years. Without one, the choice is olaparib with bevacizumab, niraparib, or bevacizumab alone. The gain is smaller when HRD is negative, and you will be told so plainly. Cost and access to some medicines vary in India. That is raised at the first talk, not at the pharmacy.

If the disease returns

Follow-up goes on after treatment ends. If the cancer comes back, the time since the last platinum chemotherapy guides the next step.

What usually happens if the cancer returns
Time since last platinumUsual next step
Six months or more.Platinum chemotherapy again, sometimes with bevacizumab. A second operation is weighed for some women, when a scoring check is in favour.
Less than six months.A different single chemotherapy drug, sometimes with bevacizumab. Mirvetuximab is an option when the tumour tests high for a protein called FR-alpha. Access in India is limited.

The tumour block is sent early for that protein test, so no time is lost.

Support alongside treatment

Palliative care is not only for the last weeks. It helps with pain, appetite, breathing and low mood. It is brought in early when strength is low, and at every return of the disease. Some problems need urgent attention the same day:

  1. A blocked bowel, with vomiting and a swollen abdomen.
  2. A tense, painful build-up of fluid in the abdomen.
  3. Sudden bleeding, or a tumour that bursts.
  4. New breathlessness, chest pain or a swollen leg, which may mean a blood clot.

Some questions are worth asking at the first visit. Is the spread IVA or IVB? Will there be a check for an operation after three or four cycles? Has the BRCA and HRD test gone? Who is your palliative care contact? Life after treatment, and the special situations that change plans, are covered on our special situations page.

Frequently asked questions

Is stage IV ovarian cancer treatable?

Yes. Chemotherapy leads, maintenance medicine often follows, and some women also have an operation after chemotherapy. What to expect in your case is discussed at the visit.

Why not operate first?

In stage IV, chemotherapy first shrinks the disease and gives the body time. The operation is then offered only if all visible cancer can be removed.

Does fluid around my lung change the plan?

It is tested for cancer cells. If they are found, that is stage IVA. Chemotherapy still comes first, and the tumour board reviews after three or four cycles.

Is palliative care a sign that treatment has stopped?

No. It runs alongside chemotherapy, easing pain, breathing and appetite. It is brought in early when strength is low.

Will I need tablets after chemotherapy?

Often, yes. The BRCA and HRD result decides which. It starts within twelve weeks of the last cycle.
