IN SHORT
Most women with ovarian cancer follow one standard plan. A few situations change the order of treatment, the size of the operation, or who else is tested.
Keeping the womb and one ovary is possible only for very early, low-grade cancer in one ovary, in a woman who clearly wants a pregnancy.
If strength or nutrition is poor, chemotherapy usually comes first. The operation follows once the body is ready, after the team reviews her again.
The usual plan is an operation to remove all visible cancer, then six cycles of chemotherapy. Some women need a different route. The table shows the main ones that Dr Swati Shah sees. Every case is first discussed at the tumour board.
| Your situation | What changes |
|---|---|
| You want a pregnancy later. | A smaller operation that keeps the womb and one ovary. Only for very early disease of certain types. |
| You are frail, older with other illness, or poorly nourished. | Chemotherapy first, with diet and exercise support. The operation follows once you are stronger. |
| You carry a BRCA change. | It decides the tablet after chemotherapy. Your mother, sisters and daughters are offered the test. |
| It is unclear where the cancer began. | The bowel and stomach are checked first. The tumour board decides before any operation. |
| You are very unwell at the first visit. | A blocked bowel, a tense swollen abdomen, bleeding or a burst tumour is seen the same day. |
| Not all the cancer can be removed. | No partial operation. Chemotherapy first, then the main operation. |
A fertility-sparing operation removes the affected ovary and tube but keeps the womb and the other ovary. The full staging steps are still done.
This is offered only when four things are all true. The cancer is in one ovary only (stage IA). It is low or middle grade. It is not the clear-cell type. And the woman has a definite wish for pregnancy. The talk about this is written down before surgery.
During the operation, fluid is washed from the abdomen and tested. Samples are taken from the lining and the fat apron (omentum). Lymph nodes are removed for testing. These steps confirm the stage. If any condition is not met, the standard operation is advised instead. Our fertility-preserving surgery page explains how women are chosen.
Ovarian cancer surgery can be long. Strength and nutrition decide whether surgery or chemotherapy comes first. Chemotherapy usually comes first if a woman spends much of the day resting, or if her blood protein (albumin) is low. The same applies to an older woman with other illnesses.
Three or four cycles of chemotherapy are given. The team then checks her again, and the operation follows if she is fit. A woman who is bedbound most of the day is first offered comfort care with the palliative team. Her wishes lead that talk.
Some women need a dietitian before any treatment. This is so if they have lost a lot of weight in recent months, are very thin, or have been eating half their usual meals for over a week. Preparation then starts from the first visit:
The BRCA and HRD tests are sent at diagnosis, not later. The result takes three to five weeks. It decides which maintenance medicine follows chemotherapy.
A woman with a BRCA change gains the most from a maintenance tablet called a PARP inhibitor. Waiting for a report that was never sent is the commonest avoidable delay. So the sample goes at the first visit.
A BRCA change can run in families. If you carry one, your mother, sisters and daughters should be offered the test. The team keeps track of this until it is done, because it is often forgotten.
Cancer in the lining of the abdomen does not always start in the ovary. It can start in the bowel, the stomach or the appendix, and that changes the treatment. Two blood markers help, CA-125 and CEA. If the CA-125 is not clearly higher than the CEA, a colonoscopy and an upper endoscopy are done first.
Cancer of the fallopian tube and cancer that starts in the lining (primary peritoneal cancer) are treated the same way as ovarian cancer. If the cancer is of the mucinous type, the appendix is also removed at surgery. Where the starting organ is still unclear, the tumour board decides before any operation. A tissue sample is always taken before chemotherapy, by a core needle or by laparoscopy. A simple fluid test alone is not enough.
A partial operation does not help. If all visible cancer cannot be removed, a sample is taken and chemotherapy is given first. The main operation follows later.
Whether all can come out is checked by laparoscopy or at an open operation, not on a scan alone. Sometimes this only becomes clear once the abdomen is open. The surgeon then stops, takes a biopsy and closes. After three or four cycles of chemotherapy, the main operation follows. For stage III disease at that later operation, heated chemotherapy inside the abdomen (HIPEC) may be added. Our cytoreductive surgery page explains who is suited to it.
If bevacizumab is part of chemotherapy, the timing matters. It is not started until four weeks after an operation. It is stopped at least six weeks before the next one.
Come to hospital the same day if you have:
Only in very early disease confined to one ovary, of low or middle grade and not clear-cell type, in a woman who wants a pregnancy. The womb and the other ovary are kept, and full staging is still done.
Age alone does not decide it. Fitness, other illnesses and nutrition do. Many older women have chemotherapy first and the operation afterwards, once they are stronger.
A BRCA change can be inherited. If you carry one, your mother, sisters and daughters have a chance of carrying it too. Knowing early lets them plan their own care.
Some cancers that look like ovarian cancer start in the bowel or stomach. When the blood markers point that way, a colonoscopy and an upper endoscopy are done before treatment is planned.
Not on the number alone. A CT scan is done, and the decision is based on you, your symptoms and the scan, not only the CA-125.
