IN SHORT

Stage III means the cancer has spread over the lining of the abdomen beyond the pelvis, or to lymph nodes. It has not reached distant organs.

Treatment has three parts. An operation that aims to leave no visible cancer, six cycles of chemotherapy, and often a medicine afterwards to hold it back.

A tumour board sets the order. If all visible cancer can be removed safely, the operation comes first. If not, chemotherapy comes first and the operation follows.

This page is one part of a larger guide. For who needs surgery for advanced ovarian cancer, how it is planned and what the stages mean, see advanced ovarian cancer surgery (stage 3 and 4) in Ahmedabad.

What stage III means, and why the order matters

Ovarian, tube and peritoneal cancer are treated as one disease. In stage III the cancer has left the pelvis. It sits on the lining of the abdomen, or in the lymph nodes at the back of the abdomen. The stage on a scan is a working stage. The final stage comes from the operation.

At this stage, three treatments work together. The question is not which one to choose. The question is which one comes first. Our staging page explains how each stage is set.

The three parts of stage III treatment
PartIts job
Operation.Remove every piece of cancer the eye can see.
Chemotherapy.Six cycles in all, one every three weeks. It treats what cannot be seen.
Maintenance.A tablet or drip after chemotherapy, chosen by a genetic test.

Before anything starts: the tests that set the order

  1. Ultrasound and a CT scan of the chest, abdomen and pelvis. The CT shows the omentum, the diaphragm, the area around the liver’s entry, the bowel and the nodes. These findings decide whether all the cancer can come out.
  2. Blood markers. CA-125, CEA and CA 19-9 are checked. If the pattern points away from the ovary, a colonoscopy and an upper endoscopy rule out a bowel or stomach start.
  3. Proof from tissue. Before any chemotherapy, a core biopsy or a laparoscopy confirms the cancer. A fluid test alone is not enough.
  4. The BRCA and HRD test. It is sent at diagnosis, not later. The result takes three to five weeks. It decides the medicine after chemotherapy.
  5. Fitness and nutrition. Blood counts, kidney and liver tests, albumin and daily activity are checked. Weight loss or poor eating brings in a dietitian.

Who decides: surgery first or chemotherapy first

The tumour board decides, never one surgeon alone. A scan is not enough to judge removal; a laparoscopy with a spread score, or an open look, settles it.

Strength counts too. A woman who is weak, has low albumin, or is over seventy-five with other illnesses usually starts with chemotherapy. She is then checked again. Our page on complete removal or chemotherapy first explains this choice in depth.

The two routes through stage III treatment
RouteWhen it is chosenThe sequence
Operation first. All visible cancer can be removed, and the body is fit for a long operation.Operation, then six cycles of chemotherapy, then maintenance.
Chemotherapy first.Cancer on the small-bowel mesentery, at the liver’s entry, or large deposits high in the abdomen. Or the body needs time.Three or four cycles, then the interval operation, then the rest of the six cycles, then maintenance.

The operation, on either route

The aim is the same on both routes: no cancer left that the eye can see. If the surgeon finds this cannot be done, a partial removal is not carried out. A biopsy is taken, the abdomen is closed, and chemotherapy comes first. Bowel may need to be removed to clear the last cancer, so consent for a stoma is taken before every such operation.

In advanced disease, normal-looking lymph nodes are not cleared. Only large, swollen nodes are removed. Our cytoreductive surgery page describes the operation itself.

HIPEC: on one route only

HIPEC is heated chemotherapy washed through the abdomen during the operation. In stage III, it is used at the interval operation, after chemotherapy, once all visible cancer is out.

HIPEC at a first operation is still being studied, and is explained as such. It is never given when cancer has been left behind. The heated chemotherapy machine at Apollo Hospital, Bhat is booked when the operation date is fixed. Read more on our HIPEC page.

Bevacizumab, and the medicine after chemotherapy

Bevacizumab is a drip added to chemotherapy when cancer was left after the operation. It starts at least four weeks after surgery. It can continue on its own for up to fifteen months. It is stopped at least six weeks before any further operation.

Maintenance starts within twelve weeks of the last chemotherapy cycle. The genetic test chooses it:

How the genetic test chooses maintenance
Test resultUsual maintenance
BRCA change found.Olaparib tablets for two years. This group gains the most.
HRD positive, no BRCA change.Olaparib with bevacizumab, or niraparib.
HRD negative or unknown.Niraparib, or bevacizumab alone. The gain is smaller, and you will be told so.

A delay here is common and avoidable. It happens when the genetic test was not sent at diagnosis. Ask at the first visit whether it has gone.

When the plan changes at once

Some problems need the consultant surgeon the same day. A blocked bowel, a tense and painful swollen abdomen, a tumour that bursts, or bleeding are among them. Once treatment ends, visits come every three months for two years. Our follow-up page covers that schedule. Questions worth asking at the start:

  1. Ask which route the tumour board chose for you, and why.
  2. Ask whether a laparoscopy is planned before the operation.
  3. Ask whether the BRCA and HRD test has been sent.
  4. Ask whether HIPEC is part of your plan.
  5. Ask whether bevacizumab will be added.

Frequently asked questions

Is chemotherapy first a sign that surgery has been ruled out?

No. On that route the operation still happens, after three or four cycles. Chemotherapy first shrinks the disease so that all visible cancer can then be removed.

How long does stage III treatment take?

Six cycles of chemotherapy, one every three weeks, plus the operation and recovery. Maintenance then follows, for up to two years with olaparib.

Why test for BRCA if no one in my family had cancer?

The result decides which medicine follows chemotherapy. It also tells your relatives whether they should be tested. It is sent at diagnosis.

Will I need HIPEC?

Only on the chemotherapy-first route, at the interval operation, and only when all visible cancer has been removed. The tumour board decides.

What if the surgeon cannot remove everything?

A partial removal is not done. A biopsy is taken and the abdomen is closed. Chemotherapy is given, and the main operation is planned after three or four cycles.
